About the Speaker(s)

Tammy Stewart is a Registered Nurse who joined Healthcare Provider Solutions, Inc. (HPS) as a Clinical Consultant in August 2021. She is a member of the National Association for Healthcare Quality and holds a Certified Professional in Healthcare Quality (CPHQ) certification. As a part of the HPS clinical consulting team, Tammy provides support and solutions to home health and hospice agencies across the country to achieve regulatory compliance.

Tammy’s commitment to service and compassionate nursing care started 30 years ago when she began providing care to patients as a home health nurse after receiving her degree from Tennessee State University in Nashville, Tennessee. Throughout her nursing career, she has worked in various capacities within the home health, hospice, and hospital industries and has participated in multiple accreditation surveys for The Joint Commission. Her experience includes successfully starting a Hospice QAPI program and serving as a Hospice Quality Manager. She has worked at the bedside as hospice nurse providing care to patients in their home and in General Inpatient (GIP) settings.

OCHCH Webinar – The HOPE Tool Part 1: General Conventions, Submission Requirements & Meeting the Compliance Threshold 

This two part program will cover the essentials of the HOPE Tool. With CY 2026 being the first full year of data collection from the HOPE tool, timely submission and accuracy of HOPE assessment documentation is essential. This session discusses general conventions of the HOPE tool including timepoints, iQIES submission requirements, submission timelines, error correction, the compliance threshold and issues that impact timely acceptance of HOPE assessments. Discussion will included guidance to accurately complete each data item.
