In this session, learn the different Home Health Quality Reporting Program and HHVBP measures tied to rehospitalizations as well as easily deployable measures that can ensure your agency is doing its part prevent the interruptions caused by them. Chronic disease management, Social Determinants of Health (SDoH), and Care Planning will be addressed as part of an overall strategy for keeping the patient at home, where they want to be.
