Rehospitalizations remain a key focus in health care quality strategy programs, including Home Health Value Based Purchasing (HHVBP). Home Health serves a high-risk population – elderly patients and those with complex medical and social comorbidities – addressing factors that contribute to rehospitalization is critical to improving your scores.
This webinar will emphasize the importance of focusing on patient mental health as a strategy to reduce rehospitalizations, a key metric that is both monitored and incentivized through HHVBP and other similar programs. By integrating mental health management into patient care, providers can not only improve overall patient outcomes but also positively impact their quality performance. The session will explore practical strategies for managing mental health conditions, a prevalent comorbidity in home health patients, and how this focus can help reduce rehospitalizations and enhance care delivery.
Learning Objectives:
- Recognize areas of policy and practice to be assessed and/or modified that will lead to better quality of care as it relates to a mental health strategy that will reduce rehospitalization rates for the agency’s population served.
- Demonstrate an understanding of how to evaluate all patients for Mental Health well being and how that will impact all other comorbid conditions.
- Distinguish status quo interventions versus best practices for Mental Health well being, that will lead to better patient outcomes, decreased rehospitalization rates, and proper payment for care in the current Home Health Care environment.
- Select areas of improvement, both personally and agency-wide, where best practices can be implemented to reduce cost, increase coordination and communication in a patient-centered care model, and improve quality of care and quality of life for the home health patients they serve.
