About the Speaker(s)

Tammy Stewart is a Registered Nurse who joined Healthcare Provider Solutions, Inc. (HPS) as a Clinical Consultant in August 2021. She is a member of the National Association for Healthcare Quality and holds a Certified Professional in Healthcare Quality (CPHQ) certification. As a part of the HPS clinical consulting team, Tammy provides support and solutions to home health and hospice agencies across the country to achieve regulatory compliance.

Tammy’s commitment to service and compassionate nursing care started 30 years ago when she began providing care to patients as a home health nurse after receiving her degree from Tennessee State University in Nashville, Tennessee. Throughout her nursing career, she has worked in various capacities within the home health, hospice, and hospital industries and has participated in multiple accreditation surveys for The Joint Commission. Her experience includes successfully starting a Hospice QAPI program and serving as a Hospice Quality Manager. She has worked at the bedside as hospice nurse providing care to patients in their home and in General Inpatient (GIP) settings.

About the Event

An effective plan of care is essential to guide care delivery and meet regulatory requirements. This session will take a deep dive into the hospice plan of care from initial development through the hospice election. The discussion will explore the development of patient-specific S.M.A.R.T. goals that address all patient problems and guide clinicians through the development of and updates to an individualized hospice plan of care to meet patient/caregiver needs throughout the hospice election.
