About the Speaker(s)
Melinda A. Gaboury, with more than 30 years in home care, has over 22 years of executive speaking and educating experience, including extensive day to day interaction with home care and hospice professionals. She routinely conducts Home Care and Hospice Reimbursement Workshops and speaks at state association meetings throughout the country. Melinda has profound experience in Medicare PDGM training, billing, collections, case-mix calculations, chart reviews and due diligence. UPIC, RA, ADR & TPE appeals with all Medicare MACs have become the forefront of Melinda’s current impact on the industry. She is currently serving as the Chair of the NAHC/HHFMA Advisory Board and Work Group and is currently serving on the Board of Directors for both the Home Care Association of Florida & Tennessee Association for Home Care. Melinda is also the author of the Home Health OASIS Guide to OASIS-E and Home Health Billing Answers, 2023.

The Hope Tool – Prepare Now for HOPE Success

The Hospice Outcomes & Patient Evaluation (HOPE) tool will be implemented on October 1st, 2025. It is imperative that hospices are prepared to capture all required data to accurately complete the HOPE Tool. This tool will replace the current HIS and will be completed at different timepoints. This session will address all data points required with the HOPE Tool, the HOPE assessment timepoints, new outcome measures that will be calculated from the data, the impact on the Hospice Quality Report Program compliance and the timeline for implementation.

- Outline the impact of HQRP compliance with the HOPE
- Define the specific items included in the HOPE assessment
- Detail the time points for completing HOPE
- Define the new outcome measures resulting specifically from HOPE data.
Hospice Conditions of Participation and Conditions of Payment – Understanding the Difference

While hospices are required to maintain compliance with the Conditions of Participation and related standards for survey readiness, hospices must also ensure documentation meets the hospice benefit coverage requirements to ensure claims are paid and/or not recouped under medical review audits. This session discusses the difference between the hospice conditions of payment and conditions of participation to ensure your agency is survey ready and ADR/medical review audit ready at all times. Discussion will provide tips for compliance, frequent denial reasons and tips for survey readiness.
