Description:

The hospice plan of care – a requirement of the conditions of participation assessed during Medicare survey, a document required for payment, and the document that is supposed to guide care provided to the patient and family. This webinar reviews the problems hospices have had with payment and on survey related to the development and review of the hospice plan of care. We delve deeper into the purpose of the plan of care and explain why it is such an important document, how to use it correctly, and how best to utilize the IDG meeting to meet requirement and improve patient care.
