OBQI & OBQM Reviews

The Outcome and Assessment Information Set (OASIS) is the group of data that is the basis for measuring patient outcomes for purposes of outcome based quality improvement (OBQI). HPS has the experience to do an in depth overview/audit of your OASIS data. This overview will provide an analysis of discrepancies and highlight points that your agency should concentrate on to ensure your OASIS data is as accurate as possible. There are three levels of auditing required for OASIS data. HPS is ready to help you implement those processes.

Outcome Based Quality Monitoring (QBQM) reports are also a key area of focus when auditing OASIS data. These reports, made available on your state OASIS transmission page, will help you to prepare for your next state survey. There are two reports: the first is the Case-Report that compares the services provided to all of your agency's patients to that of all other home health agencies across the country, the second report is the Adverse Event Outcome Report. This report contains information of adverse patient care outcomes that serves as a marker of "potential" problems in the care of a patient because of the negative nature. Investigation of a patient's care is the only way to know for sure whether the adverse event was due to inadequate care. These reports should not be taken lightly. State surveyors have been given direction to access and review your OBQM reports in preparation of your survey. Because of the availability of these reports, surveyors have been instructed to expand the scope of the case-mix stratified sample to include both current and discharged patients. HPS can review your reports, audit charts and help you prepare for state survey.
